Upon receipt of this form, your organisation is committing to one free training module for one delegate and for a Customer Visit to take place at your premises before 31st December 2011. If the Customer Visit does not take place, then Formic reserves the right to invoice your organisation for the training module. Requests for cancellation will be accepted only if received more than 20 working days before the course start date. If notice or transfer is received 20 working days or less before a course start date, or if a delegate fails to attend, then Formic is under no obligation to reschedule. Formic Ltd reserves the right to cancel a course at any time without liability. In these circumstances, delegates will be offered an alternative date. Unless otherwise stated on this form, or where parties have entered into a specific contract governing their relationship in respect of that particular purchase (such contract taking precedence), the terms and conditions of purchase associated with this order are in accordance with Formic Ltd standard terms and conditions - a copy of which is available upon request.
